Arizona: If you are a member of PacifiCare Signature Value HMO, PacifiCare Signature POS, or Secure Horizons – PBH has partnered with College Health IPA to provide outpatient services and care must be provided through College Health IPA. In addition, some products and plans may require authorization by PBH. Please call PBH at 1-800-579-5222 prior to accessing services for information on participating providers and to determine if authorization is required.
